Thank you for choosing Butler County Clinic for your medical needs. Please be sure to bring to your appointment any medications you are currently taking in their original bottles, any previous medical records you have available and your current insurance card(s). Please visit this page for a copy of new patient paperwork and bring the completed forms to your first visit. We will ask you to fill out a records release at your first visit.
All prescriptions should be reviewed with your physician at the time of your appointment. Before your appointment, please make a list of all the medicines you take, both prescription and over-the-counter, and bring it with you. If it is more convenient, just bring in your prescription bottles. If you need a prescription refill, please call your pharmacy first. Prescriptions will be refilled according to your physician’s directions.
At the Butler County Clinic, our physicians provide a variety of services such as colonoscopy and endoscopy. We also perform surgeries such as tonsillectomy and adenoidectomy, hernia repair, tubal ligation and appendectomy. We also provide colposcopy, skin lesion removal, vasectomy, spider vein treatment and a myriad of other procedures and minor surgeries.
The Butler County Clinic strives to provide access to healthcare on a 24-hour basis. We have a physician or physician assistant on call at all times. If your call is of routine nature, please call us during normal business hours. If you feel your situation is more urgent, you can speak with the provider on call by calling the hospital (402-367-1200) to determine which physician or physician assistant is on call.
From infants to elderly and obstetrics to surgical procedures, we can take care of you and your loved ones. At Butler County Clinic, P.C., we work hard to provide a full spectrum of services right here in David City so you will not be forced to travel an hour or more to receive care. We also have several specialists who provide care through our clinic to save you time and travel.
An ACO is a group of doctors and other health care providers working together with Medicare to give you better service and care. The goal of an ACO is for your doctors to communicate closely with your other health care providers to deliver high-quality care and meet your unique individual needs and preferences.
