Our Clinic is located at 336 South 9th Street in David City, Nebraska. We are on the north side of the Butler County Health Care Center. Our entrance is on the west side of the building. Our Clinic is handicapped-accessible.

Our satellite clinic, the Shelby Clinic is located at 150 N. Walnut Street, in Shelby, Nebraska. The Shelby Clinic is also handicapped-accessible.
